GRACE is flexible because no two people living with dementia are the same. Here you will find resources to support the delivery of GRACE including a video overview of each booklet.

You can access GRACE modules free of charge, if you work in the NHS or UK social care or for your own interest. They are designed to be facilitated by a person with training and supervision. If you would like training, or to use them in a different setting, please contact us: j.budgett@qmul.ac.uk.

Introduction

The facilitator works through the session one booklet to find out what is important to the client, and what support they have already. The client and facilitator then identify goals around what is most important to help the person with dementia live as well and as long as possible in their own home, and decide the goal they would like to focus on first.

These booklets look at how to support people living with dementia to make decisions about their welfare, health and finance, and make decisions on their behalf when needed.

11. GRACE last session

In the final session, the client and facilitator bring together what works best for the client, both old and new strategies. They put together a plan for the client to keep using to help achieve their goals.
